Job overview
This is an exciting opportunity to join the Ambulatory Wound Care Service.
We have various hours available dependant on the right candidate.
We are looking for some one who is passionate about wound care. Someone who works well in a team and on their own, who is flexible and adaptable and who strives to provide high quality care to patients in a clinical setting.
You will be working across all clinics in the CCICP footprint, use of a car is essential.

You will be required to carry out a variety of Health Care Assistant activities in the clinic environment. Be able to work without direct supervision under the direction of band 5 and 6 registered nurses.
Skills and knowledge of wound care, doppler assessments and leg ulcer are vital for the role, training will be given to further enhance any prior knowledge.
The successful candidate should be able to demonstrate good communication skills with patients, relatives and colleagues.

Detailed job description and main responsibilities
· Work collaboratively with the multidisciplinary team / agencies to ensure that patient needs are met.
· Involve patients, relatives / carers in the delivery of care, providing reassurance and support to assist their understanding and co-operation.
· Prepare the clinical area for safe practice by ensuring cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards.
· Collect, collate and record information, maintaining accurate patient records are kept as per trust standards.
· Participate in the delivery of patient care, following agreed plans, policies, pathways and procedures relevant to job role.
Recognise and respond appropriately to urgent and emergency situations.
Recognise any changes in patients condition or wound, ensuring it is reported to the relevant staff in a timely manner.
Enable service users wherever possible to achieve independence in looking after their health needs.
Provide written information to support this and digital information to those users with technology.
Use and maintain equipment as trained to do so and be responsible for the maintenance of the equipment ensuring it is in good working order.
To ensure stock in clinical areas is ordered to demand, stock is rotated and is stored appropriately.
· Promote the individual needs of the patient, by acknowledging preferences, rights and choices, respecting privacy and dignity, and by promoting anti – discriminatory practice, alerting senior staff if patient care appears to be disrespectful or discriminatory.
· Establish and maintain effective communication, maintaining confidentiality of information.
· Contribute to the effective management of risk, by accurate recording and reporting and correct maintenance of records and documentation.
· Ensure compliance with Trust policies, procedures and clinical guidelines, alerting senior staff if practice of self and others appear to contravene policy, or if concerned about any aspect of patient care.
· Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.
Contribute to the efficient use of stock, ensure it is rotated, ordered as demand requires and stored safely and appropriately. Make suggestions to reducing waste.
Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in Motiv8's, supervision, action learning and by maintaining a professional/personal portfolio of learning.
Take responsibility in undertaking statutory and mandatory training as required by the organisation, in addition participating in any in house training as identified by the Team Leader
