An exciting opportunity has arisen for a Physiotherapist to join our Community band 5 rotation. The post will include rotating into Planned Community Therapy, Community Integrated Neuro and Stroke service (CINSS) and our new and transformational One Coventry Integrated Team (OCIT). This role will enable you to develop knowledge and skills that support the future of community-based rehabilitation.
You will play a key role in admission prevention, hospital discharge, proactive and preventative care through rehabilitation within the community.
Responsibilities
 * Assess patients effectively, working closely with GPs, nursing services, and social care to enable an integrated care approach.
 * Develop clinically reasoned interventions that make a positive difference to the lives of the people in the community.
 * Assess patients in their own homes or in temporary/permanent residential or nursing care to maximise independence and empower rehabilitation potential.
 * Work as an effective team member; complete effective assessments of therapeutic need; establish rehabilitation prognosis; set person-centred goals; create collaborative plans enabling self-management, supporting a proactive and preventative agenda.
 * Support the unregistered workforce of dedicated Assistant Practitioners and Technical Instructors and play a lead role in their development.
 * Participate in multi-disciplinary meetings to enable the appropriate physiotherapeutic elements to be integrated in the overall client management plan.
 * Assess client understanding of treatment proposals and gain valid informed consent; respect and ensure the confidential nature of communications across organisations and with patients or their representatives.
 * Ensure that communication is appropriate to the situation.
Qualifications and Requirements
 * Full UK driving license is essential; international driving license is not accepted.
 * Successful applicants will be required to provide an immunisation and vaccination report from either their current UK occupational health provider or their UK GP.
