The Practice is looking for a Practice Nurse who can manage their own work and supervise and lead others within the team. You must be able to manage all areas of QOF and be a lead in Diabetes & Asthma.
Main duties of the job
1. You will work with the team to manage all patients on the QOF registers.
2. You will lead a small team of two newly qualified Nursing Associates and a Senior HCA.
3. You will collaborate with a complex Care Co-ordinator who supports the team.

The team comprises two GPs, an ANP, a Clinical Pharmacist, a trainee Pharmacy Technician, two ACPs, a Nurse Practitioner, two Nursing Associates, a Senior Health Care Assistant, a Complex Care Co-ordinator, two Cleaners, a Caretaker, a Business Manager, a Patient Liaison Manager, two Administrators, a secretary, and eight Care Navigators.
Job responsibilities
1. Implement processes for the effective management of patients with long-term conditions, including care for elderly and housebound patients, using evidence-based practice.
2. Work as a senior member of the multidisciplinary team to ensure nursing services are delivered effectively to the patient population.
3. Manage clinical areas such as infection prevention and control, health promotion, chronic disease management, and clinics for well women and men.
4. Support the practice management team in reviewing and delivering clinical policies and procedures.

* NMC Registered Nurse
* Experience in all areas of QOF

Salary: £37,000 to £47,000 per year, pro-rata depending on experience.
