Community Hospital health-care assistant-Surrey Downs H&C

To support and assist the senior team member in the provision of a high standard of care and the promotion of physical, mental and emotional health to individuals and client groups.

To be an effective member of the Multidisciplinary Team carrying out assigned tasks involving direct care in the support of the senior team member.

Job responsibilities

Clinical:

1. To support and assist the senior team member in the provision of a high standard of care and the promotion of physical, mental and emotional health to individuals and client groups.

2. To be an effective member of the Multidisciplinary Team carrying out assigned tasks involving direct care in the support of the senior team member

3. Undertake clinical interventions as delegated by a senior team member

Health Promotion

1. Participate in the assessment process of care needs and implementation of programmes of holistic care within set standards

2. To be involved in supporting, educating and informing family/carers where appropriate

3. Assist in the promotion of health and wellbeing of patients

Administration and Clerical Duties:

1. Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures

2. Take and record telephone messages, respond and liaise appropriately

3. Manage stationery and documentation supplies

4. To be responsible for maintaining data entry and other record keeping e.g. recording mileage and annual leave on staff databases

Team Working:

1. Work as part of the team participating in procedures and duties in accordance with agreed standards relevant to role

2. Assist with clerical and administrative duties as required

3. Maintain regular contact with the senior team member and communicate all relevant issues

General:

1. Work in partnership with patients ensuring privacy and dignity is maintained

2. Maintain close liaison with the senior team member who is responsible for the workload

3. Work in co-operation with all other professionals and agencies involved in the

4. care of the client

5. Be aware of and act upon, where necessary, the procedures in place to protect

7. Participate in the SDHC Clinical Governance Strategy

8. Participate in the SDHC Risk Management Strategy

9. Undertake such duties as may be reasonably required from time to time as are

consistent with the responsibility and scale of the post

Individuals employed to work within SDHC localities may be reasonably requested to move base temporarily.

Person Specification


Qualifications

* oBasic education to GCSE (or equivalent level)
* oEnglish and Maths
* oCare certificate


Experience

* oExperience relevant to particular post
* oPrevious Health/Social experience


Skills

* oAbility to provide quality care that is responsive to patient's/ client's needs and to work in partnership with patients and clients and other agencies
* oGood written and verbal communication
* oAbility to work effectively as part of a team, valuing contributions from team members. Ability to work independently following spoken or written instructions


Other

* oReliable
* oFlexible
* oAble to work day shifts and night shifts
