Job Overview
Band 3 Health Care Support Worker works closely with people who use our services, their families, friends and carers, contributing to how people experience our services.
They are expected to be kind, responsive, professional and informative. They play a central role in ensuring safe care and best clinical outcomes by recognising limits and working closely with registered nurses.
Responsibilities
* Safeguard people and recognise when individuals may be at risk of abuse.
* Escalate safety concerns and act as effective advocates for service users.
* Maintain accurate, legible, comprehensive records.
* Maintain compliance with mandatory training requirements.
* Provide skilled, evidence‑based care adhering to agreed policies and procedures.
* Work with patients, families and nurses to contribute to all stages of care planning, including assessing risks and needs.
* Act as patient advocates within the multi‑disciplinary team.
* Contribute to creating and maintaining high‑performing teams by communicating well with all members, understanding their roles, reflecting on one’s own practice, and identifying ways care could improve.
* Keep patients safe by recognising risks and escalating concerns when needed.
* Support continuous improvements in care and work as part of a team to deliver high‑quality, evidence‑based, holistic care.
* Advise patients and carers and signpost to supportive services.
* Participate in supervision and annual appraisals to reflect on work and monitor progress.
Experience
* Previous experience of working within a multidisciplinary team.
* Experience communicating with the general public and a range of different people.
* Previous experience working in a community setting.
* Previous experience working in a fast‑paced environment.
Qualifications
* Numeracy and Literacy skills to keep accurate records.
* Spoken and written English at an appropriate standard.
* Possession of a care certificate.
* NVQ Level 3 or equivalent (desirable).
Skills & Knowledge
* Maintaining accurate, timely, concise and legible clinical records (electronic and handwritten).
* Clear communication with the multi‑disciplinary team and effective handover.
* Ability to help assess patients, develop, implement and evaluate care plans.
* Basic knowledge of the Mental Health Act 1983 (desirable).
Working Conditions
Flexible shifts including day and weekend sessions.
