Job Summary

People who use our services are from diverse backgrounds with a wide range of needs which may include mental health, learning disabilities or dementia.

As a support worker you will be working closely with individuals, enabling them to live their lives and with support from a multi‑disciplinary team of nurses, occupational therapists, psychologists, social workers and doctors.

We will offer induction and training which will give you the skills to feel confident in your role.

Sheffield Health Partnership University NHS Foundation Trust is currently unable to provide sponsorship for this role. This is in accordance with UKVI guidelines and legislation, as these roles do not meet the skill nor salary threshold for sponsorship. If you hold a visa which allows you to work in the UK, you are welcome to apply for this role however it is important to note that SHSC will be unable to offer sponsorship after your visa expires. You must also have at least 12 months on your visa at the start of your employment for training and operational reasons.

Key Elements of the Role

* Build and maintain safe and effective therapeutic relationships with individuals.
* Support people to identify their own hopes and goals, and contribute to collaborative care/support/recovery plans.
* Support the named nurse to develop collaborative care plans and implement them creatively using own initiative.
* Communicate effectively with a wide range of people using a range of preferred methods.
* Monitor, observe and engage with people, communicating outcomes and conclusions with the rest of the team.
* Gain an understanding of a person's experiences in relation to their mental health, learning disability and/or dementia.
* Understand a person's coping strategies and strengths and support them to use these at times of distress or difficulty.
* Contribute to reflective practice and encourage compassion and understanding.
* Be able to recognise increasing risk and alert colleagues to respond as needed.
* Undertake a range of practical tasks and duties in support of the team and individual needs.
* Follow principles of diffusion and de‑escalation and where physical interventions are required, to approach in line with approved techniques considering the safety and dignity of the individual throughout.
* Be able to report untoward incidents and areas of concern as soon as they arise.
* Document any relevant information following relevant policy and legislation.
* Contribute to individual reviews and team meetings.
* Aspire to instill hope and optimism.

Responsibilities

* Implement identified plans of care for assigned patients under the direction‑supervision of qualified staff to meet the needs of the individuals identified care needs.
* Report actions and/or observations on patient care or behaviour to a qualified nurse so that a thorough evaluation of care can take place.
* Report untoward occurrences immediately to a qualified nurse so that a reassessment of care needs can be undertaken.
* Undertake general duties assigned by a qualified nurse so that high standards of service delivery can be maintained.
* Assist in planning appropriate menus, purchasing and catering in partnership with patients, to the extent required by their ability.
* Involve patients in a wide range of appropriate activities using local facilities where available.
* Encourage patient participation in day‑to‑day decision‑making, bearing in mind the principles of advocacy and self‑advocacy.
* Enable patients to maintain contact with friends and relatives by encouraging visits and assisting with letter writing.
* Accompany patients on outings and other arranged visits.
* Observe and document the patient’s participation in the care plan.
* Collaborate with the workforce regarding risk management and ensure compliance with the Trust’s policies.
* Engage in regular supervision and annual appraisal as required.
* Strengthen skills in using computer technology, basic keyboard skills and email communication.


Minimum Qualifications and Requirements

* Achieved a Grade 4 (formally Grade C) or above in Maths and English (this can include Functional Skills Level 2).
* Have fully completed the Cavendish Care Certificate.
* Can demonstrate two years’ experience of working as a Health Care Support Worker.


Person Specification

* Essential: Two years’ experience working as a Health Care Assistant.
* Good standard of written and verbal communication.
* Ability to build and sustain therapeutic relationships with patients at a level appropriate to their needs.
* Potential to organise, facilitate and develop patients’ skills in group settings.
* Consistently demonstrate non‑judgemental, respectful attitudes to patients.
* Basic keyboard skills and use of email.
* Integrity, punctuality and flexibility to work across a 24‑hour shift pattern.
* Desirable: Desire to work towards NVQ Level 3 or equivalent.
* Basic awareness and understanding of psychosocial interventions (Mindfulness, CBT, DBT, anxiety management).
* Ability to recognise and report changes in an individual’s mental health state.
