Qualified Social Worker, Community Mental Health Team
Pay rate: £32.53 per hour
Vitalis is recruiting an experienced Social Worker to work in a community mental health team in Tower Hamlets.
* Work within the Community Mental Health Team/East London NHS Trust in an integrated/partnership setting.
* Provide social work services within the framework of the Care Programme Approach, Care Act, and other legislative requirements.
* Work with individual service users in hospital and community settings, including placements, offering advice, assessment, review, and social work assistance in accordance with Social Work England parameters, aiming to improve outcomes for service users and carers.
