We are seeking a Social Worker or Occupational Therapist to join our Community Mental Health Team at the Whiteleaf Centre in Aylesbury. The role involves providing patient‑centred care to adults with mental health challenges and supporting staff development.
Responsibilities
* Work as an autonomous practitioner within a multidisciplinary team to perform comprehensive assessments in collaboration with service users and their families/carers.
* Partner with service users and their families/carers to create and execute treatment plans that ensure safety, promote well‑being, and support independence and community inclusion.
* Provide interventions following NICE guidelines to promote recovery and prevent relapse in various settings, regularly reviewing care and outcomes.
* Support and supervise junior team members, act as a mentor/preceptor to student nurses/trainees, and participate in clinical education and practice development initiatives.
* Engage in operational planning, policy implementation, and service development within the team, including leading on delegated projects and participating in research and audit projects.

Applicants must hold a full, valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required. If you are based outside of the UK you must hold SW England or HCPC registration to be considered for this role.
