The Specialist Community Forensic Team (SCFT) have anexciting opportunity for a highly motivated Occupational Therapist within ourAll-Age Forensic Community Service.
The SCFTis a multi-disciplinary team supporting transition, and continuedrehabilitation for service users, out of secure care into the community. Theteam provides service users with care and treatment pathways which offerintervention and forensic risk management. This is a regional team and operatesacross West Yorkshire.
Healthy social and occupationalnetworks outside of secure care are an important function when managing risk ina community setting. The post holder will hold a small individual caseload, havethe opportunity to complete identified Occupational therapy assessments toaddress the occupational deficits of service users and support the wider teamin formulating care, to ensure our service users have opportunities for socialnetworks and activity, and thrive in the community.
Thisrole includes working shifts including long days and mid-shifts that cover a 7-dayservice within the hours of 08:00-20:00pm. Access to a vehicleis essential.

Main duties of the job
TheAll-Age Forensic Community Service is a developing service line within theForensic Care Group. It includes:
ForensicCAMHS (inclusive of Y&H FCAMHS, Focus Secure and Focus Wakefield)
ForensicOutreach and Liaison Service for Learning Disability and/or Autism (FOLS LDA)
Our adultforensic community service (Forensic Community Transition Team (Bretton Centre)and the Specialist Community Forensic Team (SCFT))
Thevision is to create a collaborative environment between our community forensicservices and to develop and overarching clinical framework that will supportthe way in which a community forensic population is understood, supported andintervened upon, no matter what age. The purpose of the role is therefore to:
Be the lead professionalin the case management of service users.
Undertake comprehensiveassessments and risk assessments.
Be enthusiastic aboutthis service user group and motivated to support change and transition needs ofindividuals.
Able to demonstrate theability to work autonomously and collaboratively with other agencies to makeclinical judgements promoting joint decision making about care delivery toindividuals with complex presentations.
Bethoughtful and resilient in the way you engage with a multidisciplinary team.
Have strongand effective engagement skills to service users, family, friends and carers.

Job responsibilities
The post holderwill be required to work as an integral member of the multi-disciplinaryteam, providing specialist occupational therapy assessment and treatment forservice users in receipt of care from community services. Enabling service users to engage in a rangeof individual and group community based treatment activities. You will be actively involved in thepromotion of occupational therapy within the service area. The post holder will be required towork flexibly to meet the needs of the service and the client group.

1. Diploma/degree in Occupational Therapy.
2. Health Care Professions Council registration.

4. Post registration experience in mental health/learning disabilities setting.
5. Documented evidence of continuing professional development.
6. Supervision of staff and / or students.

9. Practice Education/Practice Assessor Preparation Certificate (or to achieve within 12 months).
