Health & Social Care Officer
Location: Nottingham
Rate: £18.65 per hour (Umbrella)
Hours: 37 per week
We’re hiring a Health & Social Care Officer to support adults accessing social care and health services in Nottingham. This role involves triaging enquiries, coordinating referrals, and ensuring people are connected to the right support based on their needs. You’ll work with vulnerable adults, clinicians, and partner services, often in complex or sensitive situations.
Key Responsibilities
* Manage incoming calls, enquiries, and referrals in a timely and professional way.
* Provide accurate advice and guidance based on service criteria.
* Coordinate care packages and work closely with clinicians and practitioners.
* Make informed decisions around safeguarding, risk, and complex situations.
* Support distressed callers with empathy and professionalism.
* Maintain accurate, confidential records in line with data protection standards.
* Contribute to team meetings, service development, and new staff induction.
* Assist with hospital discharge coordination for Independence Pathway services.
Requirements
* Strong IT and Microsoft Office skills.
* Excellent communication and customer service skills.
* Experience in admin, customer care, or social care, or NVQ3/Foundation Degree equivalent.
* Ability to prioritise, multitask, and stay calm under pressure.
* Confidence working with clinicians and managing complex or challenging situations.
