Job Overview
Band 6 Hospital Liaison Practitioner within the Adult Mental Health Hospital Liaison Team. Location: Woodlands Unit, Rotherham Foundation Hospital Trust Site. The role involves providing urgent and emergency response to mental health referrals, conducting full bio‑psychosocial assessments, contributing to safe discharge plans, and working closely with Trust staff to develop knowledge, deliver training, and support positive practices that place the patient at the centre of care.
Main Duties of the Job
* Provide urgent and emergency response to mental health referrals.
* Conduct full bio‑psychosocial assessments.
* Contribute to developing safe and supported discharge plans.
* Work closely with TRFT staff to develop knowledge and deliver training.
* Support positive working practices that put the patient at the centre of care.
* Serve as a competent practitioner with excellent assessment and risk assessment skills, working well under pressure and both autonomously and as part of the multidisciplinary team.
* Maintain a flexible duty rota covering the 24/7 period.
Person Specification
Knowledge
* Ability to use initiative and work without immediate supervision.
* Empathic approach to understanding patients.
* Skills in coordinating and planning interagency approaches in mental health care.
* Understanding of current mental health issues.
Qualifications
* RMN or Registered Social Worker.
* Evidence of post‑registration training.
* ENB 998 or other approved mentor qualification or willingness to complete.
Experience
* Demonstrable post‑registration experience as an RMN.
* Professional experience of working in acute adult mental health.
* Experience of MDT working.
* Skills in coordinating and planning inter‑agency approaches in mental health care.
* Experience of assessing and managing risk.
Skills
* Excellent report/assessment writing skills.
* Ability to work as part of a multi‑professional team.
* Understanding of the CPA process.
* Understanding of the importance of user/carer involvement in care planning.
* Experience and understanding of Safeguarding responsibilities.
