Overview
BYRON PCN includes the 4 GP practices of Oakenhall Medical Practice, The Om Surgery, Torkard Hill Medical Centre and Whyburn Medical Practice, located in the Hucknall area of Nottingham. We have an opportunity for a Student Nursing Associate to join the PCN to support primary care nursing across practices. The role is based within GP practices, delivering nursing clinics and working with the Practice Nurse teams, with occasional home visits expected.
We are seeking a Student Nursing Associate who is enthusiastic about delivering high quality care in primary care. Applications from those willing to undertake the Nurse Associate Foundation degree programme and with experience as a Health Care Assistant (or similar) will be considered. The postholder should be hard-working, committed to educational development, highly motivated and capable of working independently as well as in a team. You will be clinically supported by Practice Nurses and the PICS Lead for PCN nursing.

Job responsibilities (detailed)
Being an accountable professional
* Act professionally at all times and use knowledge and experience to make evidence-based decisions and solve problems.
* Recognise and work within the limits of competence, maintaining professional boundaries and taking responsibility for actions.
* Proactively develop knowledge and skills, attending statutory/mandatory updates as required.
* Work as part of the GP Practices / PCNs multidisciplinary team to provide and monitor care under direct and indirect supervision.
Promoting health and preventing ill health
* Support patients to improve and maintain mental, physical, behavioral health and wellbeing, including health checks and signposting to other services.
* Contribute to long-term condition management for diseases such as respiratory disease, diabetes, CHD, renal failure, and hypertension.
* Encourage participation in health screening; when trained, perform cervical screening tests.
* Care for individuals with complex conditions (e.g., dementia, mental health conditions, learning disabilities).
Providing and monitoring care
* Carry out procedures safely in PCN practices and in patients’ homes (including care homes): clinical observations, venepuncture, spirometry, ECG, diabetic foot checks, sample processing, wound care, medication administration, injections (SC/IM) including immunisations, cervical cytology screening (after training), minor surgery assistance, and ongoing reassessment referrals.
Working in teams
* Recognise changing priorities and seek advice from registered nurses or other professionals as appropriate.
* Support, supervise and mentor learners and new care staff; participate in team meetings and clinical supervision.
Improving safety and quality of care
* Apply infection prevention and control principles, maintain a safe environment, clean and restock equipment, and participate in audits and training (hand hygiene, vaccine cold chain, etc.).
Contributing to integrated care
* Maintain clear, concise records and communicate across MDTs to support integrated care across health and social care sectors; manage risks and safeguard vulnerable individuals; support learners and keep knowledge up to date.
Supervision
* Ensure access to appropriate clinical supervision within the PCN and a named adviser for day-to-day support.
