Job Description: Virtual Ward Nurse/Practitioner
Join the Urgent Community Response Team / Virtual Ward, providing seven-day-a-week rapid assessment and intervention for adults of all ages. The service caters to individuals who do not require specialist acute hospital admission but need crisis intervention within a community setting due to physical, psychological, or social changes. These may include temporary alterations in their well-being or social circumstances requiring short-term rehabilitation or intervention.
Virtual wards offer acute clinical care at home for a short duration as an alternative to hospital care, utilizing a step up/down approach. Patients in a virtual ward are reviewed daily by a Geriatrician or Advanced Clinical Practitioner via a digital platform that enables remote monitoring and escalation to a multidisciplinary team.
The post holder will participate in a seven-day rota, including weekends and public holidays, conducting assessments, determining care levels, initiating care plans to ensure patient safety, and supporting the virtual service delivery within patients' homes.
Main Responsibilities:
1. Develop appropriate boundary competencies (nursing/therapy) to assist the multidisciplinary team in managing ongoing clinical risks.
2. Manage referrals into the Urgent Community Response Team/Virtual Ward, liaising and coordinating staff, and managing service capacity.
3. Provide short-term interventions to maintain patient safety at home or prevent re-admission, such as equipment provision or domiciliary care, under Geriatrician-led oversight.
4. Work autonomously within Trust policies and professional standards, maintaining accurate and comprehensive documentation.
5. Determine clinical diagnoses, develop and evaluate individualized treatment plans, and liaise with other agencies to ensure appropriate care pathways and communication.
