Community Diabetes Specialist Nurse

We are seeking an experienced and motivated Community Diabetes Specialist Nurse to deliver expert, evidence‑based care across our Integrated Diabetes services across Leicestershire, Leicester, and Rutland. You will provide leadership, manage a caseload of complex patients, and work collaboratively with multidisciplinary teams to ensure integrated care pathways are implemented effectively.
Work independently while supporting the team in managing clinical caseload within your specialist field. Continue responsibility for assessment of health needs and development and evaluation of standards of care.
Main duties of the job
Clinical Care & Support
* Provide expert assessment, advice, and treatment for people with diabetes across the community, and outpatient settings.
* Manage your own patient caseload, delivering individualised care plans that promote optimal health outcomes.
* Offer ongoing education and support to patients and families to enhance self‑management and quality of life.
Education & Training
* Teach and train patients, carers, and healthcare staff, fostering confidence in managing complex conditions.
* Provide educational resources and programs tailored to different learning needs and clinical settings.
Collaboration & Integrated Care
* Work closely with multidisciplinary teams—to ensure coordinated, holistic care.
* Act as a key point of contact for complex cases, ensuring effective communication and continuity of care.
Service Development & Quality Improvement
* Conduct audits, collect data, and apply research findings to improve standards and clinical outcomes.
Leadership & Professional Development
* Support team growth by sharing expertise, mentoring colleagues, and promoting best practices.
* Take an active role in clinical governance, policy development, and innovation in care pathways.
Job responsibilities
This job covers the community setting therefore must be able to work at multiple locations and have own transport.
Person Specification
Training and Qualifications
* Post‑basic Specialist Qualification
* Recognised Specialist Qualification
* Teaching qualification
* Nurse prescriber or willingness to complete training
* Knowledge of common conditions related to diabetes and its complications, its management and treatment
* PGCert or equivalent in diabetes
Knowledge & Experience
* 5 years Post‑Registration experience
* Clinical experience & specialist knowledge of diabetes
* Evidence of leadership skills and willingness to become an autonomous practitioner
* Evidence of continued professional development
* Good working knowledge of relevant IT
Communication and Relationship Skills
* Written and interpersonal skills
