Care Plus Groups AdultLearning Disability Services includevarious specialist teams, including: a Community Learning DisabilityTeam (CLDT) to support the physical health of learning disabled adults & Carers Support needs; Psychological Therapy Service to support the mentalhealth needs of learning disabled individuals; an Intensive Support Team (IST)to support individuals with complex needs and behaviours of concern; an AdultAutism (no-LD) Service (AAS) and ADHD Service, which provides both diagnosticassessments and a full range of post-diagnostic support; and a ForensicOutreach Liaison Service (FOLS) to support autistic and/or learning disabledadults who are at risk of offending or who have an offending history to livesafely within their local community.
All our Adult NeurodiversityServices follow a proven person-centred, needs-led approach to deliveringsupport to individuals, their families, carers, supporters and other servicesand professionals involved in their care.
The Service works in collaboration and in partnership with otherlocal health and social care providers, including Navigo to deliver co-producedstatutory, User-Led and Peer-to-Peer Support services to NE Lincolnshire & across the Humber Transforming Care Partnership (TCP) area,which comprises of Hull, East Riding of Yorkshire, NorthLincolnshire and NE Lincolnshire.

Health and Wellbeing Co-ordinator
hours per week - Fixed Term for 12 Months

Salary Range 4 £23,360 - £25,784 per annum you will be appointed on afixed salary point within the range
We are seeking a highly skilled and motivated individual to join ourTeam to maintain and drive the development of high-quality services for peoplewith a learning disability. This is a rare and excitingopportunity to become part of a motivated and passionate multi-disciplinary CommunityLearning Disability Service.
As a Health and Wellbeing Coordinator you will be responsible forsupporting people with learning disabilities to get their health needs met. Youwill be responsible for liaising closely with other health professionals,developing health action plans, supporting people to attend healthappointments, health screening and healthy activities. In addition, you willalso support people with learning disabilities and their carers to understandgood health and wellbeing, you will also support mental capacity and bestinterest decision making processes.
You must be well motivated, a self-starter andpossess an NVQ 3 in health and social care or equivalent and have a minimum of2 years experience of working with people with a learning disability.
