Urgent Community Response - Advanced Clinical Practitioner

It's an exciting time for our Urgent Community Response (UCR) Service, including the General Virtual Wards Caseload, as we continue to build on our successful model. UCR services work with patients in their homes across West Sussex communities to avoid hospital admissions and support those being discharged home.

The Service provides clinical interventions (including intravenous therapy) from a team of Medics, Nurses, Paramedics, Physio, and Occupational Therapists for admission avoidance pathways (2-hour response and general virtual wards), in line with the national specification for UCR.

Operating 365 days per year, the service receives referrals from various health and care professionals, triages, assesses, and delivers care to ensure patients can remain safely at home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8 am and 8 pm.

We are seeking an Advanced Clinical Practitioner (Nurse or Allied Health Professional) to join us. If you're a dynamic, patient-focused individual looking to take a lead role in a team that provides cutting-edge, excellent acute community care, we'd love to hear from you!


Main duties of the job

The postholder will work autonomously as an Advanced Clinical Practitioner, assessing, investigating, diagnosing, treating, and managing patients with complex and diverse factors. They will develop and maintain skills beyond normal practice, such as advanced physical assessment, diagnostics, and prescribing. The role involves planning and directing patient management, accessing services within the multidisciplinary team and broader health, social care, education, vocational, and voluntary sectors. They will lead on the strategic development of Trust-wide services in their specialist area and support education programs for clinical staff.

The postholder will provide peer support, assist in service planning and development, and ensure compliance with legal and regulatory frameworks through local governance procedures.

Job responsibilities

The postholder will be competent in establishing protocols and guidelines for service growth and supporting research for the development of their specialty.

Person Specification


Qualifications and/or Professional Registration

* MSc in Advanced Clinical Practice or equivalent at MSc level in clinical practice at level 7
* Registered Health Care Professional qualification/degree appropriate for ACP role
* Current Registration with NMC / HCPC
* Evidence of recent CPD in relevant field
* Independent/Supplementary Prescribing (HCPC V300) or Advanced Medicines Management module
* Advanced Physical Assessment course Level 7
* Coaching or Mentoring qualification
* Teaching qualification (e.g., PGCert)


Experience

* Substantial post-registration experience in relevant clinical field
* Leadership/Management experience at band 7 or above
* Experience in teaching and mentoring staff across multidisciplinary teams
* Experience in clinical steering groups/taskforces at organizational level
* Experience within clinical or professional groups/networks at ICS or national level
* Experience leading and delivering quality improvement


Skills and Knowledge

* Up-to-date advanced clinical skills in relevant specialty
* Ability to deliver evidence-based care for complex and deteriorating patients
* Excellent communication and presentation skills
* Strong leadership skills for engaging and implementing change
* Ability to develop partnerships across organizations and local healthcare system
* Ability to manage high-pressure situations and prioritize workload
* In-depth knowledge of NHS and health and social care legislation relevant to community services


Other Requirements

* Willingness to undertake training for role development
* Valid UK driving license and access to a vehicle for work
* Flexible working approach

This role is subject to DBS checks as per the Rehabilitation of Offenders Act (Exceptions Order) 1975.

Salary: £53,755 to £60,504 per annum
