The City District Nursing Team is offering a permanent Band 6 Community / District Nurse contract.

You will be equipped for mobile working and will be provided with a laptop and smartphone, meaning that you will have access to patient data, enabling you to complete documentation and paperwork efficiently, in real-time. The District Nursing Service aims to provide an excellent standard of evidence-based care with the patient always at the centre. As a service we can also provide unplanned nursing intervention to patients in their homes and are part of an integrated service that supports patients to remain in their home environment.
You will be one of 5 Sisters / Charge Nurses in the team all hold a caseload of no more than 150 patients. Responsibilities
* Manage a caseload with appropriate supervision and support, ensuring safe, effective patient care in the community.
* Utilise mobile working tools (laptop and smartphone) to access patient data and complete documentation in real-time.
* Provide unplanned nursing interventions to patients in their homes as part of an integrated service to enable patients to remain in their home environment.
* Collaborate with the District Nursing Service and other partners to support patients across Herefordshire and adjacent areas.
* Contribute to the development of clinical competencies and leadership skills within the team.
