This is an exciting opportunity for an experienced Adults Social Worker to join a dynamic Adult Social Care service in Lewisham. You will play a key role in supporting vulnerable adults, promoting independence, and ensuring safeguarding responsibilities are met in line with statutory duties. What you will do Complete Care Act 2014 assessments and reviews using a person-centred approach. Carry out Mental Capacity Act assessments and make Best Interests decisions when required. Manage a varied caseload of adults with complex needs, including older people, those with physical disabilities, and individuals with mental health needs. Develop, implement, and review personalised care and support plans. Lead safeguarding enquiries in line with Section 42 requirements. Work collaboratively with health professionals, care providers, and partner agencies. Support safe and timely hospital discharge processes and community-based interventions. Maintain accurate, detailed, and timely case records in line with statutory guidance. Promote independence, wellbeing, and positive outcomes while managing risk effectively. Requirements Essential: Qualified Social Worker (DipSW, CQSW, BA/MA Social Work). Registered with Social Work England. Minimum of 2 years post-qualification experience in Adult Social Care. Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures. Experience managing complex caseloads and producing high-quality assessments and reports. Desirable: Experience working within a Local Authority Adult Social Care team. Familiarity with systems such as Mosaic or Liquidlogic. Experience in hospital discharge or integrated teams.
