Our client is looking for an experienced Population Health Management Analyst
To provide dedicated analytical capacity to embed Population Health Management (PHM) approaches across the London Borough of Bexley and the Integrated Care System (ICS) at place level. The postholder will bridge public health intelligence and NHS data-led service planning, enabling the design, targeting, and evaluation of interventions that reduce health inequalities, improve outcomes, and enhance patient experience. They will use existing datasets to support segmentation, forecasting, and equity audits, while preparing the system for future linked data capabilities.

Direction
Support the development of PHM methodology in line with local, regional, and national priorities.
Ensure PHM intelligence aligns with Joint Strategic Needs Assessment (JSNA) priorities, NHS planning, and ICS strategies.
Promote collaboration across council, NHS, and voluntary sector partners to maximise shared learning and avoid duplication.
Implementation
Deliver analytical projects including population segmentation, equity audits, and service redesign intelligence.
Translate public health and JSNA insights into actionable PHM approaches.
Develop automated reporting pipelines (e.g. dashboards) for real-time monitoring.
Provide specialist analytical and epidemiological advice to public health, ICB partners, and service leads.
Join current workstreams (e.g. frailty, long-term conditions) and contribute PHM insight.
Lead intelligence-informed projects in prevention, access, outcomes, and forecasting.
Map local data availability and support readiness for data integration across health and care.
Organisational Control and Development
Ensure effective data governance, confidentiality, and compliance with IG standards across council and ICB datasets.
Contribute to building analytical maturity across the system, advocating for PHM capabilities.
Maintain knowledge of PHM methods, including risk stratification, impact ability modelling, and equity analysis.
Engage in continuous professional development and maintain awareness of national PHM and health intelligence developments.
Promote equality and inclusion in all work, ensuring services are designed to meet the needs of disadvantaged groups.
Ensure compliance with statutory requirements, organisational policies, data protection, and confidentiality principles.
Staff Management & Development
While no direct line management is expected initially, support training and upskilling of colleagues in PHM methodologies.
Share best practice and build analytical capacity across both council and NHS teams.
Personal Effectiveness
Communicate complex findings clearly to both technical and non-technical audiences, including senior leaders and elected members.
Work flexibly across organisational boundaries, fostering strong relationships with partners.
