The post holder will be the clinical lead for the Virtual function of the Home First Service for SDHC.

Qualifications
Essential criteria
* Registered first level nurse/practitioner with relevent health degree
* Completed Master's Degree in Advanced Practice or working towards
* Non- medical independent prescribing for healthcare professionals or working towards
* Advanced clinical practice skills/ competencies
* Management of patients with acute medical illness
* Clinical examination skills
* In-depth knowledge in their specialist clinical field
Desirable criteria
* Demonstration of Leadership or management experience
Experience
Essential criteria
* Significant post registration experience
* Leadership in quality initiatives such as clinical benchmarking, service development
* Significant experience in audit and research
* Healthcare professional prescribing
Skills
Essential criteria
* Clinical leadership skills
* Communication skills, both written and verbal
* Negotiation and conflict management skills
* Change management
* Car driver with current UK driving license and access to car for work
