This role involves supporting our clinic duties alongside key-working a varied yet manageable case load.
About Us
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.

Job responsibilities
Under a multi‑disciplinary framework you will undertake assessments and coordinate the care of patients with both functional and organic mental illness. You will support or lead aspects of our busy community clinics as well as key‑work a caseload which will involve ongoing assessment, formulation, care planning, risk assessments, completing reviews and offering tailored interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies where necessary.
Excellent communication, report writing and assessment skills are essential for this post. Applicants should have the appropriate qualifications, a comprehensive working knowledge of mental health disorders and of the Mental Health Act.
Qualifications
* Appropriate Professional Qualification
* Evidence of continuing professional development
* Evidence of CMHT experience
* Minimum of 2 years post qualified experience working in a community mental health setting at band 5 or 6
