Specialist Mental Health Primary Care Practitioner

We are seeking a Mental Health Nurse, Social Worker or Occupational Therapist team leader to join our developing primary care Mental Health Team in the East locality of Southampton. You will be based within GP surgeries and working with service users of adult working age including older persons with mild to moderate mental health difficulties. You will work within scope of practice; if you have worked within specialities, these skills will be transferable.
Main duties of the job
You will be a clinician contributing to the triage service, offering early assessment, brief interventions and signposting for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within the teams. Required: good communication skills and ability to build positive partnerships with key stakeholders in primary and secondary care. You will work collaboratively with Solent Mind and have peer recovery workers work alongside practitioners in the surgery.

Salary: £49,387 to £56,515 a year (based on full time hours)
