We’re looking for an Triage & Support Officer to join our One You Lincolnshire service. One You Lincolnshire is an Integrated Healthy Lifestyle Service which has been supporting local residents, since July 2019, with Smoking Cessation, Weight Management, Alcohol Reduction, Physical Activity and Health Coaching. More recently the service has expanded to offer Child and Family Weight management, and Strength and Balance for adults aged 65. Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths. A typical day as an Triage & Support Officer might include : Use a variety of systems and approaches to ensure clients get the best, positive and client-centred approach when supporting them through our onboarding process. • To offer guidance and support to clients both on the phone and in written format to ensure they are best aligned to the most appropriate health intervention offer for them and their needs. (Training will be provided if successful). • To contact clients daily who wish to start their journey with us, being a friendly, warm and empathic voice that represents our service positively and to correctly record their information on our bespoke systems accurately and timely. • To respond to requests for information from clients, partner organisations and the public. Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources! Ensuring due consideration is given to our Safeguarding, Information Governance and Clinical Governance policies and procedures.
