* Provide visible clinical and operational leadership to the Children's Community Nursing Team across Gloucestershire.
* Lead the delivery of high-quality specialist community nursing care for children and young people with complex and acute health needs, including palliative and end of life care.
* Oversee referrals, triage processes, caseload allocation and workload management to ensure equitable and responsive service delivery.
* Support children and families to access care closer to home, reducing hospital admissions and facilitating safe discharge from acute settings.
* Manage and supervise nursing staff, including recruitment, appraisal, absence management, mentoring, professional development and clinical supervision.
* Ensure robust safeguarding practice and promote high standards of clinical governance, risk management and patient safety.
* Lead service improvement initiatives, audit activity, evaluation and implementation of innovative and digitally enabled care pathways.
* Support transition planning and collaborative working across children's and adult services
* Build strong partnerships with paediatricians, acute trusts, education providers, social care, commissioners and voluntary sector organisations.
* Investigate complaints, incidents and service concerns
* Deputise for the Head of Children's Community Nursing Team when required.
* Participate in occasional out-of-hours/on-call arrangements to support children receiving end of life care at home.
