An opportunity has arisen to join our nursing team. Together with our experienced practice nurse and HCA you will deliver high quality care to a lovely patient population. We are based in a modern medical centre and are a long established training practice.

Job responsibilities
Experienced nurse needed to join our mutually supportive practice. Experience working in a GP team isn't essential but desirable. Three years post graduate nursing experience is essential.
This requires a working knowledge of:-
* Undertake appropriate management of long term conditions, screening programmes, immunisation programmes within their level of competency and with adequate and recognised training.
* Chronic disease management (desirable)
* Ensure accurate completion of all necessary documentation associated with a patients healthcare.
* Perform home visits where appropriate.
* Responsibility for reviewing and acting on own sample results or those referred onto you from another nurse or associated healthcare professional, or referring to a more senior clinician when appropriate.
* Providing nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols.
* Support and promote the development of team working to ensure a high standard of nursing care within the treatment area.
* Adhere and work to defined polices and protocols, including health and safety, equality and diversity, and confidentiality within the practice
* Assist in the ordering and maintaining of stock
* Be aware of national standards of infection control and good practice guidelines and active observation of current working practices across the practice ensuring that procedures are followed.
* Actively identifying, reporting and correction of health and safety hazards.
* Responsible for the safe use of all diagnostic and treatment equipment, and alert other team members to issues of quality or risk.
* Attend practice meetings whenever possible and those that are mandatory.
* Maintain competency and develop new skills as decided by service requirements.
* Communicate, refer and liaise effectively with other agencies and staff regarding patients well being, with appropriate regard to confidentiality.
* Effectively manage own time, workload and resources.
The successful candidate will be friendly and approachable and enjoy working in a richly multicultural community. He or she will help our practice to continue to develop through working in close co-operation with the other team members. The candidate will embrace the changes occurring in general practice and be keen to help enact those changes for the betterment of our patients. It is essential to have highly developed organisational skills to meet admin demands and a refined practical knowledge of the mechanics of modern GP nursing such as liaising with outside agencies.

* Demonstrate an understanding of practice processes and change management

* Local referral routes.
* Languages relevant to our community such as urdu, punjabi or bengali
