An exciting opportunity for a Clinical Pharmacist has arisen to join our Walnut Medical Centre Practice. Previous experience working on site in a general practice is essential. There may be room for negotiation on the terms and salary depending on experience. The post is fully based at Walnut Medical Centre, although remote working for some sessions is possible. The successful applicants will work closely with the existing clinical teams to deliver patient care, working innovatively and collaboratively.
The number of hours is flexible and may be shared with more than one applicant.
This will be a combination of on-site and remote working.

Completion of CPPE and Independent Prescriber qualifications are an advantage and would be preferred.

Main duties of the job

Main duties include delivering expertise in clinical pharmacy, providing clinical pharmacy input to the teams based at Walnut Medical Centre, assisting in delivering the practice's vision, collaborating across multidisciplinary networks, supporting the clinical team in pharmacist management, and delivering requirements of the DES and practice pharmacy requirements. Skills in undertaking audits, quality improvement projects, and knowledge of Eclipse are desirable.
About us
The Clinical Pharmacist will support the delivery of practice targets and PCN objectives, managing long-term conditions, and supporting patients directly. Key qualities include building effective relationships with patients, GPs, nurses, community pharmacists, hospital and practice staff, and working effectively as part of a team. Demonstrable pharmaceutical skills in core areas, including long-term conditions, and an understanding of general practice are essential. The post holder will be responsible for chronic disease management and clinical medication reviews to proactively manage patients with complex polypharmacy.
Job responsibilities
The post holder will manage chronic disease areas, undertake clinical medication reviews, engage in medicines optimisation, and support the management of medicines at discharge from hospitals. Responsibilities include reviewing medication needs, supporting medicines safety, managing repeat prescriptions, conducting audits, implementing guidelines, and supporting public health campaigns.
Additional duties involve supporting practice staff with prescription queries, managing medication reviews for various patient groups, providing medicines support clinics and helplines, and ensuring compliance with CQC standards.

Qualifications
* Must have experience working within general practice
* Degree in Pharmacy (MPharm/BPharm)
* GPhC Registered
* Full driving license
* CPPE
* IP qualified or working towards an IP qualification
Desired experience
* Member of the RPS
* Clinical Diploma
* Experience with clinical systems (SystemOne, Emis, Vision)
* Experience in delivering and monitoring practice and PCN targets/medications
