Purpose of the Role
To provide high-quality, person-centred support to young adults with Learning Disabilities and Autism in the community and, as services expand, within Supported Living environments.
The role focuses on:
Enabling independence
Teaching and developing life skills
Supporting positive behavioural outcomes
Promoting community inclusion
Ensuring safe management of health conditions (including epilepsy and diabetes)
Delivering detailed, outcome-focused documentation aligned with PBS and regulatory standards
You will work with increasing autonomy but remain accountable to senior staff and the Registered Manager.
Key Duties & Responsibilities
1. Person-Centred Support & Independence
Deliver support in line with each individual's Person-Centred Plan, PBS Plan and Risk Assessments.
Support individuals to develop:
Cooking and meal preparation skills
Budgeting and money management
Travel training and community navigation
Personal care and hygiene skills
Social and communication skills
Promote choice, control and independence in all daily activities.
Support individuals to access community services, education, volunteering and social opportunities.
Assist individuals to attend centres such as Mudlarks, Church Farm and Jackie's Drop-In.
Support transitions into Supported Living and help individuals adapt to more independent environments.
2. Positive Behaviour Support (PBS)
Implement Positive Behaviour Support strategies as outlined in Behaviour Support Plans.
Accurately document behaviours, triggers, interventions and outcomes.
Use proactive and preventative strategies to reduce incidents.
Support emotional regulation and communication development.
Work collaboratively with families, professionals and multi-disciplinary teams.
3. Clinical & Health Support
Support individuals with Epilepsy, including:
Monitoring seizure activity
Following seizure management protocols
Recording incidents accurately
Support individuals with Diabetes, including:
Monitoring blood glucose levels (where trained)
Supporting dietary requirements
Recognising signs of hypo/hyperglycaemia
Administer or assist with medication in line with LSR Care policies.
Undertake required training and refresher updates.
Report any changes in health, wellbeing or behaviour immediately.
4. Safeguarding & Risk Management
Understand safeguarding procedures and act in accordance with LSR Care and Local Authority policies.
Identify potential signs of abuse or neglect and report appropriately.
Follow all risk assessments and health & safety guidance.
Ensure safe use of equipment and environments.
Report incidents, accidents and near misses promptly.
5. Documentation & Professional Standards
Maintain clear, detailed and professional daily records.
Record progress towards outcomes and independence goals.
Contribute to care reviews and support planning meetings.
Ensure documentation reflects PBS principles and CQC standards.
Comply with GDPR and confidentiality requirements.
6. Communication & Teamwork
Communicate effectively with:
Service users
Families and carers
Health and social care professionals
Internal management team
Attend:
Supervisions (minimum 6 per year)
Team meetings
Annual appraisal
Mandatory training sessions
Act as a positive ambassador for LSR Care Limited.
7. Expansion into Supported Living
As LSR Care expands into Supported Living, Support Workers will:
Support individuals in shared or individual tenancies
Encourage greater independence in household management
Promote tenancy responsibility and independent living skills
Maintain high documentation standards suitable for regulated housing-based care
Adapt to working both community-based and within supported living properties
Person Specification
Essential
Minimum 1 year UK care experience (preferably with Learning Disabilities and Autism)
Full UK Driving Licence (Held for a Minimum of 1 Year)
Experience supporting individuals with SEND needs
Understanding of Autism and person-centred approaches
Willingness to undertake PBS, Epilepsy and Diabetes training
Strong communication and documentation skills
Flexible, reliable and professional
Desirable
NVQ/QCF Level 3 in Health & Social Care
Experience within Supported Living
Experience implementing Positive Behaviour Support Plans
Experience supporting epilepsy and/or diabetes
Knowledge of CQC regulations
Higher rates of pay may apply for Level 3 qualified and experienced staff.
Suitability & Professional Conduct
Work in accordance with all LSR Care policies and procedures.
Maintain confidentiality at all times.
Inform senior staff immediately if unable to attend a shift.
Engage actively in professional development.
