The Amyotrophic Lateral Sclerosis multidisciplinary clinic is a busy and vibrant ambulatory clinic in the Department of Neurology that is also heavily focused on research, both clinical and observational. The devoted and close-knit team consists of 10 physicians,5 nurse practitioners, 9 nurses, 4 physical therapists, 3 speech pathologists, 2 respiratory therapists, clinic coordinators, and several research study coordinators.
Minimum of two years RN experience required.
The Registered Nurse of the Home-Based Program will perform home visits and work closely with the Neurological Clinical Research Institute (NCRI) and ALS Clinic leadership to expand access to care and help plan, refine, and extend care activities beyond the walls of the clinic. The Registered Nurse will work with the NP and MD in the Home-Based Program to help ensure continuity of care for patients who have enrolled in the Home-Based Program. The RN will collaborate closely with Compassionate Care ALS (a non-profit organization focused on supporting people with ALS in their homes and a partner in the Home-Based Program) and the patient's health care team (including MGH, community, and home health providers) to more fully integrate care and improve patients' experience.
The ideal incumbent is flexible, independent, and creative. Individuals must have strong written and verbal communication skills and the ability to converse comfortably with patients and their caregivers, in addition to health care providers.
Clinical Responsibilities:
· Collaborates with provider(s) to deliver targeted healthcare services to patients with motor neuron disease.
· Provides clinical home visits and appropriately notifies the provider with updates on the patient.
· Obtain pertinent information about the patient's condition and symptoms, and provide timely feedback to the provider.
· Provide support and assistance with challenging patient/physician situations.
· Explore innovative ways to care for homebound patients.
· Maintains tracking system to determine program effectiveness, with support from internal data management and IS.
· Meets routinely with NCRI and ALS clinic leadership to report on activities, patient satisfaction, problems, and recommendations.
· Meets monthly with Medical Directors of the Home-Based Program to review progress and overall performance of the program.
· Helps to continuously refine the implementation of the program to find the most useful means to see people with ALS both on a routine basis (for those who have difficulty leaving their homes), and when we are most needed (for our general ALS clinic population).
· Works with outside facilities and clinical staff. This may include hospitals, PCPs, VNA, Hospice, and private help.
· Maintains familiarity with the activities and interests of the NCRI and MGH ALS clinic.
· Promotes and maintains a close partnership between the MGH ALS Clinic and Compassionate Care ALS (CCALS).
· Works alongside the patient services coordinator to fully implement the House Call program, and develop an approach to increase the volume of patients using the program over 1-3 years
Relationship Management: Is responsible for developing and maintaining clinical relationships with patients, caregivers, care teams, outside physicians, nurses, and ALS advocacy groups.
· Routinely communicates with NCRI leadership to establish effective networking.

· Actively participate in network development initiatives and communicate initiatives back to the access team for relationship building within MGH affiliate networks.

LICENSES:
Basic licensure as a registered nurse in the Commonwealth of Massachusetts.
Basic Life Support (BLS) current
EDUCATION:
Bachelor's Degree in Nursing
EXPERIENCE:
Minimum of 2 years relevant nursing experience
Previous ALS experience preferred
Previous home care experience preferred
WORKING CONDITIONS:
Ability to work independently in the home setting
Must be able to and have the means to regularly travel locally for clinical home visits
24hr/wk position within structure of normal business hours, Monday - Friday, 8:30-5pm (with possible exceptions for attendance at conferences or other visits outside of business hours)
