Job summary
Care Plus Group Primary Care practicesprovides the first point of contact for registered patients accessing thehealth care service. The service will work collaboratively with patients toassist them in managing and preventing illnesses and coordinating access tospecialised care.
The Care Plus Group Primary Care practices,Open Door and Quayside, provide services for a client group with a range ofdiverse and sometimes complex needs including substance misuse, homelessness,mental health problems and self-neglect.

Job Title: Bank Advanced Nurse Practitioner
Hours: Bank hours to cover maternity leave
Monday Friday
Salary dependent on experience you will be appointed on afixed salary point within the salary range.
You will act as an autonomous practitioner workingindependently and in conjunction with other healthcare professionals within a range of primary care facilities to assess, diagnoseand treatthe conditionsof thosepatients attending theservice.
You will provide expert professional advice to patients,carers and colleagues and ensure the maintenanceof clinical excellence and will undertake Clinical Nursing Practice at anadvanced level using expert knowledge and clinicalskills to deliver holistic care topeople accessing services.
You will assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis and complex needs. Clinically examine and assesspatient needs from a physiological and psychological perspective, plan clinicalcare accordingly and deliver care according to NICE guidelines. Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).
You will provide professional and qualityadministrative support and reception service to patients, doctors, other healthprofessionals and all visitors and work under general supervision and withinwell established procedures.
