Main duties of the job
As a Rotational Clinical Pharmacist, you will work within various clinical specialties, such as:
* Acute Medicine
* Cardiology
* Endocrine
* Respiratory
* General & Emergency Surgery
* Care of the Elderly
You will collaborate closely with the wider healthcare team to ensure medicines optimization and safety throughout patient stays. Leadership and prioritization skills are essential to provide high-quality pharmacy services to our trust.
Your daily tasks will include medicines reconciliation, patient counseling, medicines information, discharge planning, and clinical reviews.
You will also work within our Homecare, Oncology, and TPN services.

Job responsibilities
Band 6
Provide supervised pharmacy services to wards and departments, ensuring safe, cost-effective medicine use according to departmental procedures. Responsibilities include:
* Taking drug histories on admission
* Reviewing medications for safety and efficacy
* Developing and updating pharmaceutical care plans
* Monitoring drug therapy and advising on dose adjustments
* Patient education and counseling
* Implementing local and national guidelines
* Challenging prescribers when necessary
* Communicating medicine-related information effectively
* Preparing injections and infusions correctly
* Independent prescribing for medicines optimization

Band 7
Deliver clinical pharmacy services to wards and departments, ensuring safe, cost-effective medication use. Key duties include:
* Medicines reconciliation on admission
* Reviewing and updating care plans
* Therapeutic drug monitoring and dose adjustment
* Patient education and counseling
* Developing treatment protocols
* Implementing guidelines
* Analyzing drug charts and advising on medication selection
* Challenging prescribers when appropriate
* Communicating effectively with patients, relatives, and healthcare professionals
* Preparing injections and infusions

Person Specification
Experience and Knowledge
* Registered Pharmacist
* Committed to professional development
* Strong clinical pharmacy knowledge
* Master's degree in pharmacy (MPharm) or equivalent
* At least 6 months pre-registration hospital experience
* One year post-registration experience in hospital, community, or GP practice
* Effective communication skills
* Ability to work independently and in teams
* Organizational and time management skills
* Ability to handle stressful situations
Disclosure and Barring Service Check
This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A disclosure submission will be necessary.
Salary: £37,338 to £52,809 per annum (pro rata).
