This is a Primary Care role servicing the Northeast, Central and West and South Primary Care Networks made up of 12 member GP Practices.

The role will develop according to our patients needs and with the overall leadership of our clinical colleagues.

A key part of these roles will be to support individuals to get to the right services and to feed back to the wider project team with information which will in turn support their health care plans to improve overall health and wellbeing.


Main duties of the job

Key Duties and Responsibilities

- Listen to patients in an empathetic and non-judgemental manner and let them know about available services.

- Once patient has identified their preferred option help them to access the required service if needed.

- Be available for follow up calls with patients if they need further signposting

- Keep accurate records/data collection as required by project leaders

- Participate in regular team meetings to ensure information about all services is documented and shared.

- Provide specific support to the Social Prescribing Team and Mental Health Practitioners in supporting their clients.

- Be pro-active in building good relationships with practice colleagues, other service providers including community mental health team and the voluntary sector.

We provide Primary Care Mental Health Services to the Northeast, Central and West, and South Primary Care Networks, covering a registered population of circa 142,300 patients.

Please note, whilst this role will work across our 3 PCN member practices; A designated lead practice will be appointed as the employing organisation for this post.


Job responsibilities

The following are the core responsibilities of the Mental HealthAccess Co-Ordinator. There may be on occasion, a requirement to carry outother tasks; this will be dependent upon factors such as workload andstaffing levels.

The Mental Health Access Co-Ordinator is responsible for:

a)Listening to patients in an empathetic andnon-judgemental manner and let them know about available services.

b)Once patient has identified their preferredoption, help them to access the required service if needed.

c)Follow up calls with patients if they needfurther signposting.

d)Data entry/data allocation and collation;processing and recording information in accordance with services procedures.

e)Participating in regular team meetings toensure information about all services is documented and shared.

f)Providing specific support to the SocialPrescribing Team, Link Workers and Mental Health Practitioners in supportingtheir clients.

g)Being pro-active in building goodrelationships with practice colleagues, other service providers includingcommunity mental health teams and the voluntary sector.

h)Undertaking a variety of administrativeduties to assist in the smooth running of the service, including clericalsupport to clinical staff and other members of the practice team.

i)Filing patient records and correspondence inpatient medical records.

j)Facilitating effective communication betweenpatients, members of the primary health care team, secondary care and otherassociated healthcare agencies.

k)Support practices to proactively identifypatients registered with a Severe Mental Health Illness (SMI) and work withpractice and patients to increase the uptake and quality of SMI PhysicalHealth Checks.

e) Excellent timemanagement skills ability to prioritise, plan and deliver work items inline with set expectations.

h) Confidence toescalate or refer workflow where needed and identify risk.

i)Confidently communicate at all levels to include MedicalProfessionals, Commissioners and Governing bodies (where appropriate).

j)Build working relationships quickly and effectively within our MemberPractices and external stakeholders instilling confidence and maintainexisting relationships.

k)Always maintain strict confidentiality and awareness of GDPR/ datasharing throughout working practices.

l)To provide cover for members of the teamduring periods of sickness and annual leave.

m)To attend Training courses and training daysas requested.

a) An awareness andunderstanding within Mental Health/ Wellbeing. (Either as a service userand/or supporting within this remit/ via previous work /voluntary or personalexperience

b)Exposure to working within a healthcare environment (or equivalent)

c)Previous knowledge of working with SystemOne and/or any other PrimaryCare IT packages.

d)A full current driving licence and access to a vehicle will be useful


Person Specification


Qualifications

* GCSEs at grade C or equivalent (or 4-9), including English and Maths

Experience

* Experience of working in a medical or primary care setting
* Computer literate and good knowledge of MS Office and Outlook
* Self-motivated and proactive
* Awareness of Data Protection Act and need for confidentiality of Information
* Attention to detail, able to work accurately, identifying errors quickly and easily
* Excellent understanding of data protection and confidentiality issues
* Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals, with the ability to adjust communication styles
* Ability to maintain good working relationships with staff at all levels
* Committed to own continuing personal development and an ability to support others to develop and progress
* Ability to sustain concentration with constant interruptions
* Ability to maintain administrative systems that will underpin good practice
* Experience of working in a medical or primary care setting
* Experience of working in mental health
* Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines


Physical Requirements

* Commit to a DBS Check
* Able to undertake the demands of the post with reasonable adjustments if required
* UK Driving Licence
