We are excited to offer a Specialist Physiotherapy opportunity within our Long-Term Conditions Rehabilitation Service, designed to transform patient care and outcomes. Our service brings together Specialist Falls, Cardiac Rehabilitation, and Moving Forward (previously Long Covid) teams to deliver truly holistic, patient-centred care. Working collaboratively, we support individuals with a wide range of long-term conditions to improve their quality of life. In this role, you’ll focus on empowering patients through effective symptom management, helping them take control of their health, wellbeing, and recovery. Using a variety of therapeutic approaches and self-management strategies, you’ll play a key role in enabling lasting, meaningful change. You’ll be involved in shaping and developing the service, delivering both one-to-one and group interventions, and working within a forward-thinking multidisciplinary team committed to high-quality, evidence-based practice. Advert You will be an enthusiastic and motivated individual with broad experience in a range of therapy clinical settings, with the ability to demonstrate excellent communication and interpersonal skills To provide highly specialist assessment, diagnosis, and treatment of patients with complex therapy needs as an autonomous practitioner within our newly developed symptom led Long-Term Conditions Rehabilitation team. To manage a defined caseload using evidence based / patient centred principles to assess, plan, implement, evaluate, and record all interventions. To deliver virtual and face to face groups which educate patients about living with a long-term condition and treating the symptoms and/or associated needs. To provide one to one clinic sessions where patients require a more intensive approach to care. To work as part of an MDT and use an evidence-based approach to managing patients with a range of conditions using a symptom led approach. To provide clinical education and training to others, including therapy colleagues and students.
