End of Life Quality Improvement Facilitator

We are seeking two motivated and compassionate end‑of‑life quality improvement facilitators to join the Stockport Specialist Palliative Care Service. These roles offer flexible working options, including full, part‑time, or a combination of hours tailored to suit the right candidates. This is an exciting opportunity to play a key role in improving the quality of life care across Stockport.


Main duties of the job

* Act as a role model and champion for palliative and end‑of‑life care.
* Support the delivery of palliative and end‑of‑life care training across Stockport’s health and social care system.
* Contribute to the planning, development, and delivery of high‑quality training programmes promoting best practice and improving patient care.
* Use quality improvement methodologies to support a range of initiatives across both hospital and community settings.
* Assist with quality improvement projects aimed at enhancing patient and family experience.
* Work collaboratively with multidisciplinary teams to embed evidence‑based practice.
* Support the development and dissemination of resources, tools and guidance to improve standards of care.
* Actively participate in clinical audit activities and support the monitoring and evaluation of service improvements.
* Undertake a patient‑facing/bedside clinical educator and facilitator role for staff development, specifically relating to general care of the dying person.
* Work alongside colleagues in the Stockport Specialist Palliative Care Service.
* Share lessons learned from significant events to inform changes in practice.
* Develop, coordinate and deliver appropriate training plans required to implement palliative/end‑of‑life care principles that reflect national guidance.

Job responsibilities

* Lead and support palliative/end‑of‑life care education and training across Stockport health and social care ecosystem.
* Use a quality improvement approach to facilitate hospital and community projects to develop end‑of‑life care.
* Monitor end‑of‑life care against national and local guidance and standards.
* Facilitate the delivery of high‑quality end‑of‑life care across all settings, coordinating services and advanced care planning.
* Develop links at both strategic and operational levels to ensure a supportive and coherent approach to the implementation of palliative/end‑of‑life care principles.
* Work with other facilitators and the specialist team to lead implementation of palliative/end‑of‑life care principles, including local care homes.
* Facilitate Stockport’s participation in national, regional or local initiatives and audits regarding palliative/end‑of‑life care.
* Encourage and share good practice, ensuring lessons learned are used to inform policy and guidelines.
* Develop internal and external networks with professional colleagues to disseminate best practice across relevant organisations.
* Foster team working across professional and organisational boundaries.
* Identify training needs and competency requirements of staff relating to palliative and end‑of‑life care.
* Provide leadership and advisory direction in all matters pertaining to palliative/end‑of‑life care education and training.
* Undertake patient‑facing/bedside clinical educator support for the general care of the dying person.
* Plan and organise appropriate education and training programmes, including primary care meetings, care of the dying, electronic palliative care coordination, advance care planning, communication skills, and mandatory end‑of‑life care training.
* Ensure new documentation is endorsed by organisations across the health and social care ecosystem.
* Attend and participate in local, regional and national conferences.


Person Specification

* Education and Qualifications
o Registered Healthcare Professional
o Degree level education or equivalent
o Teaching Qualification
o Evidence of post‑registration study in palliative care or end‑of‑life care
o Masters or level‑7 study
o Quality Improvement qualification or relevant experience
* Knowledge
o Experience of caring for the dying person in hospital or community settings
o Research, development and/or clinical audit experience
o Awareness of national/regional strategic guidance and plans
* Experience
o Experience at senior HCP level
o Clinical experience of palliative and end‑of‑life care
o Experience delivering/facilitating face‑to‑face education
* Skills and Abilities
o Ability to work independently and as part of a team
o Ability to plan own workload
o Effective time management
o IT literate; demonstrate use of IT tools and packages
o Effective and confident presenter; use facilitative approach
o Flexible approach to work
o Ability to travel between different bases
