POSITION: Support Worker

We are currently looking for people to join our team who are committed to meeting the needs of all the people we support, have a good sense of humour, and are friendly towards others. Our specialist Supported Living and Outreach service in Maidenhead provides support for people with Learning Disabilities. Responsibilities
* Promote and deliver a person-centred support culture across the service
* Support people with all aspects of their daily living as per their support plan, including personal care and medication management
* Support and coach individuals to learn independent living skills such as cooking and laundry tasks
* Assist people in accessing community activities for social, educational, or employment opportunities
* Safeguard service users’ rights and dignity
* Participate actively in support plans and risk assessments
* Maintain effective communication within the team and attend regular team and service user meetings
* Liaise professionally with family members and other professionals
