Nottingham University Hospital's NHS Trust (NUH) has an opportunity for an enthusiastic, dynamic, and highly motivated individual to join the Acute Medicine Physiotherapy Team at Queen's Medical Centre (QMC) as a Band 4 Physiotherapy Assistant Practitioner. This is a part-time role (18.75 hours per week) on a 12-month fixed-term contract or secondment.
Acute Medicine is a fast-paced specialty with high patient acuity and turnover. You will manage your own caseload, perform new patient assessments, and formulate discharge plans to support patient flow through the hospital. The role primarily involves working on the front door, Ward B3, and the Acute Medical Receiving Area (AMRA), with some opportunities to support the physiotherapy team on the Level 1 Unit with early mobilisation and rehabilitation of patients in the acute phase.

1. Managing a caseload of patients on a short-stay medical ward with support from physiotherapists.
2. Assessing patients' mobility and discharge suitability, often involving frailty, falls, care home patients, and less complex medical cases.
3. Participating in delegated rehabilitation interventions, including involvement in reviewing progress and supporting rehabilitation for patients with simple upper/lower limb trauma, stroke, neurology, or respiratory conditions, as assessed by a qualified physiotherapist.
4. Attending multidisciplinary board rounds focusing on patient flow and discharge, advocating for patients.
5. Ordering and collecting mobility aids to ensure adequate supply.
6. Recording patient contacts accurately on clinical management systems.
7. Updating Nervecentre with mobility status and discharge plans, and sending referrals promptly.
8. Sending community referrals for patients at risk of falls or with reduced mobility/function.

The Physiotherapy service is part of the Therapies, Outpatients and Phlebotomy Services (TOPS) Directorate within the Specialist Clinical Services care group.
