What you will do
* Develop and implement care plans tailored to individual patient needs, focusing on manual handling, rehabilitation, and safe discharge planning.
* Collaborate with a multidisciplinary team to ensure comprehensive patient care.
* Provide education and training to other staff members on safe manual handling practices.
* Conduct patient assessments and contribute to the acute medicine team in implementing effective rehabilitation strategies.
* Maintain accurate and up-to-date records of patient progress and therapy sessions.
Requirements
* You must be registered with the HCPC to apply for this role.
* A UK driving licence is essential, as you will need to travel between different hospital sites and community settings.
