The Role
This post is a NMAHP post rather than a Nurse Consultant post and focuses on improving the prevention and management of frailty across all care settings. Working within a newly developed frailty clinical network the post holder would represent all NMAHPs in developing and reviewing frailty pathways that provide positive patient experience and outcomes. They would be required to develop advanced clinical assessment and prescribing competence if not already obtained within the first year.
The post holder will provide professional leadership, strategic direction and expert consultancy on all aspects of practice and care of frail people across the patient pathways in Lanarkshire. This will involve working across professional and organisational boundaries, in partnership and in collaboration with others including service users, carers, social work, medical staff, nursing, allied health professionals, voluntary sector staff, and local education providers.
There would be no direct line management responsibilities.
Additional Information Specific to the Role
The key outcomes from the post will include achievement of;
1. Improve use and reporting of clinical frailty screening tool across all services
2. Further development and systematic spread of integrated models of care for frail people.
3. Support development of sustainable models of care which provide alternatives to admission to acute and residential/ nursing home care
4. Improve access to specialty bed within 24 hours of admission where admission to secondary care cannot be avoided
5. Support reduced length of stay for inpatient settings
6. Increase in the number of people discharged home with flexible community packages of care or support services in place
7. Support delivery of ward flow bundle and PDD across inpatient settings
8. Support review and development of future clinical models for all bed based services across Lanarkshire
9. Support development of locality response
10. Support implementation of whole system frailty strategy
11. Develop model and deliver NMAHP bed holding capabilities in offsite areas
12. Support collaboration with Frailty ihub
13. Support Frailty GIRFE Pathfinder project
14. Support implementation of My Health, My Care, My Home

What we'll need you to bring
Knowledge and Professional Training
15. Registered Nurse or AHP with appropriate professional registration
16. Significant post registration experience
17. Completed advanced clinical assessment / V300 or willing to complete within first year
18. Masters degree or evidence of attainment of Masters level education (SQF level 11), working towards achievement of the full Masters degree
19. Experience in change management / quality improvement
20. The post holder is required to demonstrate a high level of competence and excellence in professional leadership, expert practice, strategic planning / service development, education and training, and research and development
21. Working knowledge of relevant legislation, guidelines and strategies.
Experience
22. Extensive clinical experience in the care and treatment of frail people
23. Experience of specialist interventions in the assessment and management of frail people with acute and community settings
24. Experience of practice and service development
25. Experience of teaching / education of others
26. Evidence of high level leadership skills in delivering major change in clinical/service practice
27. Experience in research methodology and delivery

28. Fixed Term or Secondment for 10 months

Please note this is a fixed term post for 10 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location
This role will be based in Nursing Directorate within Netherton House, University Hospital Wishaw.
