We are working with a client in Crewe who is seeking a Social Care Assessor to support vulnerable adults and their careers. The role involves completing needs-based assessments, coordinating care packages, and managing a caseload in line with legislation and local policies. Key Responsibilities Complete needs-based assessments and develop individual Care Plans Manage and review care packages, responding to changing needs Act as Care Manager, coordinating services and support Liaise with families, professionals, and external agencies Maintain accurate and up-to-date case records Ensure safeguarding and risk management procedures are followedKey Requirements NVQ Level 3 in Social Care (or equivalent) Experience working with vulnerable adults in a social care or health setting Knowledge of safeguarding, risk assessment, and care planning Ability to work collaboratively with multi-disciplinary teams Good communication, organisation, and IT skills Flexible approach and ability to travel within the boroughContract Length: 3-6 months with a view to extend to an ongoing contract.
