Contract role – Leaving Care Team
Pertemps are recruiting for an experienced Personal Advisor within Children’s Social Work in the Telford area.
This is to work in a Leaving Care Team, and you will need to have experience in a similar role, including the following job responsibilities:
1. Undertake the statutory role of Personal Adviser role for individual Care experienced young people from age 16 up to the age 21 or 25.
2. To be the key professional and case manager to Former Relevant care experienced young people and when needed to those young people who are Relevant or Qualifying for advice and assistance
3. To work with young people leaving care and their support network to support the development of independent living skills and ensure a smooth transition to independence
4. To assist and support young people to prepare for leaving care and living independently; providing support, advice and guidance to them up to age 25.
5. You will need to be a driver with access to your own transport
