Community Practitioner

North Tyneside Recovery Partnership have an exciting opportunity for a Band 6 nurse to join the team, working into the Physical Health Pathway

The role entails working with those affected by substance & alcohol misuse with associated and additional physical health needs.

The role entails providing safe and effective interventions, harm reduction advice and use of psychosocial interventions to support service users to consider and achieve recovery.

The role involves individualised clinical assessment, care planning, formulation, and collaborative working across the multi‑agency team.

The team consists of a well‑established mix of professionals that work collaboratively to deliver individualised care.

The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students.


Main duties of the job

* To be responsible for the Assessment and Management of casework of service users who have substantial complex & physical health needs.
* Provide physical health wellbeing checks including taking of bloods, observations and having an outreach approach.
* Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.
* In collaboration with the Service User & Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
* To deliver evidence based Clinical and Psychologically based Interventions.
* The post holder will work as a member of the MDT ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
* Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
* To provide compassionate care that is based on empathy, kindness, respect and dignity. To incorporate a client centred philosophy into nursing practice and client centred collaborative care.

Person Specification


Education and Qualification

* Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
* Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
* Proven track record of post registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Post Registration Qualifications in Psychological Interventions
* Experience of working within a community team.
* Non‑Medical Prescribing


Skills and Competencies

* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management / Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
* Ability to work in a team and lone working


Knowledge and Experience

* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
* Experience of working with those who have complex needs
