An exciting opportunity has arisen for an assistant practitioner to join our Community Rehabilitation Teams in Kirkcaldy. The role focuses on delivering therapeutic treatment programmes to adult service users in their own homes, preventing hospital admission and supporting early discharge. The post holder will also maintain an autonomous caseload, assessing, progressing and discharging patients within agreed parameters.
Responsibilities
* Work with Occupational Therapy and Physiotherapy staff as part of the Community Rehabilitation Team to deliver therapeutic treatment programmes.
* Assess and manage an individual caseload, progressing and discharging service users autonomously within agreed parameters.
* Provide therapeutic support to older people, the primary group of service users referred to the team.
* Collaborate with a multidisciplinary team, demonstrating excellent organisational and communication skills, and work independently when required.
* Be available to travel throughout the Fife area and occasionally work at other community hospital sites.
* Participate in regular supervision, PDP and training opportunities.
* Work during public holidays as required.
Qualifications
* HNC/HND in Health and Social Care or equivalent, or SCQF Level 8 (e.g., Postural Stability Instructor) or equivalent qualification.
* Experience as a therapy support worker essential, particularly with older people.
* Ability to travel throughout the area and work autonomously within parameters.
