LDAP Keyworker Co-ordinator/Business Support

18.75 hours per week. You will work alongside the Clinical Team Manager to support the Key Worker Pilot, an NHSE/I funded pilot to improve outcomes for CYP with Learning Disabilities &/or Autism. You will lead on the evaluation of the project & be responsible for the administrative tasks for the clinical staff through effective organisation & co-ordination.

You will collate & report the clinical data & KPIs required, offer project lead/business support to the Clinical Team Manager through coordinating action plans, supporting the implementation of governance processes, collating performance data & supporting the team in the development & operational effectiveness of the new service

They will support the clinical team to assist in tasks which impact on the effective running of the service that might prevent them from undertaking their clinical duties. These tasks will include maintaining the Dynamic Support Register, coordinating clinical meetings & appointments.

The role will manage new referrals, assisting in the process to determine whether the CYP meets the criteria for the keyworker role co-ordinate the team to run efficiently. This will require excellent communication skills & the ability to form good working relationships between agencies.

Sitting within the CAMHS Admin team &will have line management & supervisory duty for administrative staff in the service.

All Livewell Southwest staff are expected to able & willing to work across a 7 day service.


Main duties of the job

To assist the Keyworker pilot by offering project lead & business support. To support governance, performance & reporting processes. To collate the relevant information required in a format that can be presented to stakeholders. Managing communication on behalf of the team, taking referral information, coordinating the teams clinical activity, arranging multi-agency meetings, co-ordinate & facilitate the writing of clinical reports & onward referrals

Assisting with the management of the Dynamic Support Register (DSR) with the designated lead

To provide line management & supervision to administrative staff within the CAMHS Admin team

In addition, the coordinator will undertake the administrative tasks for the team. This will include managing calls, typing where required & support the clinical team completing tasks

The coordinator will be responsible for developing a process to co-ordinate an urgent response to a situation, where a clinical staff are required to respond within a certain timeframe, or if a crisis situation arises

The coordinator will be expected to attend meetings relevant to their role, for instance attending housing meetings, risk management meetings & CPA reviews as required

The worker will ensure that quality & performance monitoring data collated and available monthly

The coordinator will ensure that quality monitoring for the team is delegated & carried out as required

Job responsibilities

To assist the Clinical Team Manager in the implementation stageof the Keyworker pilot by offering project lead and business support. Thiswill involve undertaking tasks to operationalise the team. To supportgovernance, performance and reporting processes. To collate the relevantinformation required in a format that can be presented to stakeholders. Managingcommunication on behalf of the team, taking referral information, coordinatingthe teams clinical activity, arranging multi-agency meetings, co-ordinate andfacilitate the writing of clinical reports and onward referrals.

Assisting with the management of the Multi-AgencyDynamic Support Register (DSR) with the designated lead. This will involvekeeping it up to date, reporting the relevant information for monitoringpurposes and ensure that there is oversight of all the CYP open to the team.

To provide line management and supervision toadministrative staff within the CAMHS Admin team, ensuring staff aresupported and are up to date with mandatory training, induction, includingprobationary periods and annual appraisals.

In addition, the coordinator will undertake the administrativetasks for the team. This will include managing incoming calls, typing whererequired and will support the clinical team completing tasks that free uptime to care. The coordinator will be expected to demonstrate excellent andsupportive customer care during any call, particularly when assisting withthe needs of families who contact the team and other multi-agency professionals.

Thecoordinator will be responsible for developing a process to co-ordinate anurgent response to a situation, where a clinical staff are required torespond within a certain timeframe, or if a crisis situation arises.

Co-ordinatethe oversight of CYP in out of area care, either in Tier 4 beds orresidential care.

The coordinator will be link with ProviderCollaborative work and be responsible for being aware of the bed availabilityacross the southwest. They will be aware of all out of area placements usingthe virtual ward on system one and will ensure this is kept up to date, witha regular review of progress of the person and their plan for repatriation.

The coordinator will be aware of CYP imminentdischarge plans and will assist in supporting the clinical team to implement planswhere they are able.

The coordinator will be expected to attend meetingsrelevant to their role, for instance attending housing meetings, riskmanagement meetings and CPA reviews as required.

The worker will ensure that quality and performancemonitoring data collated and available monthly.

The coordinator will be expected to attend meetingsrelating to the keyworker service development or incident follow up and willtake initial steps to following up incidents and ensuring good relationsbetween agencies.

The coordinator will assist in service changes,training and development of others.

To know how to report accidents and untoward incidents and to doso promptly.

To obtain knowledge of Trust policies, procedures and practicesand carry out duties in accordance with them.

To be aware of legal issues and have an understanding of theMental Health Act, Mental Capacity Act and Transforming Care.

Thecoordinator will ensure that quality monitoring for the team is delegated andcarried out as required.

Thecoordinator will be able to access staffing information for the team and be ableto maintain e-roster.

Toassist with keeping an up to date record of patient receiving care outside ofPlymouth.

To be willing to participate actively in appropriate in-servicetraining programmes which the manager believes to be of benefit to theindividual. To assumeco-responsibility for maintaining skills and knowledge levels.

Undertake specific responsibilities designated by the ClinicalTeam Manager.

To undertake mandatory and other training as directed by theTrust.

To participate in continuing role / personal development usingthe appraisal system.


Person Specification


Qualifications

* NVQ 4 in care or any comparative Health & Social Care qualification
* Level 4 project management qualification (or willingness to undertake)


Experience

* Previous experience in delivering care to those who have been diagnosed with LDAP and Mental Health issues.
* Experience in working within a multi- disciplinary setting.
* 2 years at Band 3 or equivalent.
* Line managing junior staff
* Experience in care settings for LDAP
