The Mental Health Support Worker plays a vital role in providing person-centred care and practical support to individuals experiencing mental health challenges. Working as part of a multidisciplinary team, the post holder will support service users in their recovery journey, helping them to build independence, resilience, and improve their overall wellbeing. The role involves delivering day-to-day support tailored to individual care plans, including assisting with daily living activities, promoting social inclusion, and encouraging engagement with therapeutic and community-based services. The Mental Health Support Worker will develop positive, professional relationships with service users, offering emotional support while maintaining appropriate boundaries. The post holder is responsible for monitoring and reporting on individuals' mental and physical wellbeing, identifying any changes or risks, and escalating concerns in line with safeguarding and organisational policies. Accurate record-keeping and effective communication with colleagues, healthcare professionals, and external agencies are essential aspects of the role.
* Provide person-centred support in line with individual care and recovery plans.
* Assist service users with daily living activities, including personal care, meal preparation, and maintaining a safe living environment.
* Offer emotional support and encouragement to promote independence, confidence, and wellbeing.
* Support individuals to engage in social, recreational, and community-based activities.
* Monitor and observe service users' mental and physical health, reporting any changes or concerns promptly.
* Maintain accurate, timely, and confidential records in accordance with organisational policies.
* Work collaboratively with healthcare professionals, families, and external agencies to ensure coordinated care.
* Promote safeguarding by recognising and responding to signs of risk, abuse, or neglect.
* Support service users in managing medication where appropriate and in line with training and guidelines.
* De-escalate challenging situations and respond appropriately to behaviours of concern.
* Encourage and support service users to attend appointments and access relevant services.
