Overview

This post will be based at Acklam Medical Centre. Greater Middlesbrough PCN is looking for a dynamic pharmacist to join our multidisciplinary team to deliver medicines optimisation services. An exciting opportunity has arisen to join our network; if you are interested in developing your skills in primary care then we would love to hear from you.

The successful applicant should be able to demonstrate excellent clinical and communication skills and have experience of implementing medicines optimisation, quality and patient safety initiatives and resolving prescribing issues. You must be a Registered Pharmacist with the General Pharmaceutical Council, ideally with Independent Prescriber status. You must also be computer literate, organised and able to work independently as well as part of a team. You will prioritise workload, build key relationships and be confident interacting with a wide range of stakeholders such as GPs, Nurses, secondary care clinicians and patients.

About Us: The Greater Middlesbrough PCN supports over 70,000 patients from 8 practices across the area. The PCN consists of multidisciplinary teams including Clinical Care Coordinators, Care Coordinators, Clinical Pharmacists, Mental Health Practitioner, First Contact Physio and Social Prescribing Link Workers. SystmOne and EMIS are used for clinical systems. We are an active network that works with local voluntary organisations and partners to develop new models of care.

Responsibilities

* Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
* Be a prescriber, and work with and alongside the general practice team.
* Manage care for patients with chronic diseases; undertake clinical medication reviews to proactively manage complex polypharmacy (elderly, residents in care homes, multiple comorbidities including frailty, COPD/asthma, learning disabilities or autism via STOMP).
* Provide specialist expertise in medicines use while addressing public health and social care needs and tackling inequalities.
* Provide leadership on person-centred medicines optimisation and quality improvement; contribute to the QOF and enhanced services.
* Deliver structured medication reviews to maximise adherence, reduce waste and promote self-care.
* Support integration of general practice with wider healthcare teams to improve outcomes and manage workload.
* Develop relationships with other pharmacy professionals across PCNs and the wider health and social care system.
* Play a central role in shared care protocols, clinical research with medicines, liaison with specialist/ community pharmacists and anticoagulation services; engage in clinical supervision.


Collaborative Working Relationships

* Recognise roles of colleagues and communicate to gain cooperation from patients and professionals across NHS/private organisations.
* Refer to appropriate colleagues when necessary and maintain effective working relationships within and outside the practice.
* Liaise with CCG Medicines Optimisation Team and other stakeholders for consistent patient care.


Knowledge, Skills and Experience

* Experience with common long-term conditions in general practice.
* Minimum of 3 years postgraduate experience in pharmacy.
* Holds or works towards independent prescribing qualification.
* Plan, manage, monitor and review pharmaceutical care programmes across core disease areas; deliver professional expertise and direct service provision.
* Ability to interpret options, solve problems, and refer to GPs/ANPs when appropriate.
* Adhere to legal, ethical, professional and organisational policies; involve patients in decisions as per NICE guidelines.


Leadership, Management and Professional Development

* Demonstrate governance understanding and contribute to workplace vision; pursue continuous professional development.
* Understand national priorities and resource utilisation; comply with standards of practice and performance management.
* Ensure clinical supervision and participate in appraisal; stay up to date with clinical practice.


Health, Safety, Equality and Confidentiality

* Comply with Health and Safety policies and data protection requirements; maintain patient confidentiality at all times.
* Uphold equality and diversity policies in employment and practice.
* Travel between practices/sites as required for the role.


Qualifications

Essential

* Master’s degree in pharmacy or equivalent; registered with GPhC; postgraduate diploma and qualifications for specialist knowledge.
* Experience of medication reviews and patient counselling; experience working with clinicians in multidisciplinary settings; experience in clinical audit.
* UK driving licence and car access.

Desirable

* 2 years relevant post-qualification experience.
* Independent Prescriber status or commitment to undertake the course.


Other Information

* Disclosure and Barring Service checks may be required (rehabilitation of offenders act); standard DBS checks will be undertaken as part of employment.
* UK professional registration is required.
