The role

Roles and Responsibilities:

* To abide by the NICE guidelines in relation to Administration of medication and all its coverage

* Support Workers are to practice the 5 principles of the Mental Capacity Act at all times

* Being actively involved in partaking of Key worker role and responsibility and ensuring guidelines to this role are appropriately followed

* Responsible for achieving service objectives delivered by Managers and Senior managers embedded within Team meetings, supervisions, workshops, and training delivery

* To take responsibility for attending any communication briefs such as, Team meetings, supervisions, workshops, and training delivery

* To perform basic personal care tasks as outlined in the individual Support plan ensuring the support provided is dignified, respectful and above all person centred

* To alert the Deputy manager/ Service Manager of any changes in the individual’s condition or circumstances

* Responsibility to ensure that the right level of support and care is delivered continually paying explicit detail to the support plan provided

* Delivering a person-centred approach abiding by the Support plans for direction and delivery of a tailored support package bespoke to the individual

* Responsible for meeting individuals commissioned hours are adhered to as part of ensuring good quality care provided

* Ensuring all risk management, risk assessments are reviewed and maintained monthly in association with the individuals within the service working in partnership with the relevant circle of support identified

* To ensure that Report writing, and Record keeping is maintained and written in a manner that is dignified, accurate and jargon free at all times (incident forms are sent to management in a timely manner as soon as an incident happens so actions can be taken to support and mitigate risk)

* To actively support those, we provide support to partake in design, schedule and deliver any associated activities or, engagements arranged for, on, or behalf the individual
