Are you a qualified Physiotherapist looking for an exciting new opportunity?
As part of Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will work within the Dementia Outreach and Therapy teams to assess care home residents with dementia and create treatment plans. Your role will focus on falls management, mobility, and manual handling, supporting independence and aiming to reduce hospital admissions. ICHS aim is to improve residents and staff experience by delivering high-quality, multidisciplinary care in an integrated model, with referrals prioritised to meet individual needs. Caseloads are primarily adults with dementia in care homes. Applicants should demonstrate strong Physiotherapy skills and experience supporting people living with dementia, ideally within community settings.
Professional development is actively encouraged, with opportunities to collaborate with specialist services and access ongoing education. Team members benefit from regular therapy meetings, peer support, and supervision.
The Dementia Outreach and Therapy teams play a vital role in optimising wellbeing through person-centred rehabilitation. By joining, you will gain valuable experience managing complex cases and contribute to improving the quality of life for vulnerable adults.
We also encourage interested applicants to shadow the service for a day to see the role in action and its positive impact on patients. This Physiotherapy role is part of the Dementia Outreach (DOT) element of the Integrated Care Home Service, which includes dementia specialist therapists, RMNs, and Assistant Practitioners. DOT therapists work alongside physical therapy clinicians as an integrated team, supported by a therapy lead. Referrals are triaged and assigned to the most appropriate therapist.
The DOT Physiotherapist must demonstrate advanced clinical knowledge and experience in dementia care. They will complete holistic assessments, prescribe tailored therapy programmes, and manage their own caseload. The role also includes supervising Assistant Practitioners and acting as a resource for the wider service.
As an integrated service, collaborative working is central. ICHS Therapy team and wider therapy meetings are held monthly, for therapists from across CityCare to share expertise and develop innovative practice. The Physiotherapist will contribute to service evaluation, audits, meetings, and working groups, representing the service where required.
The team supports delivery of the Enhanced Care in Care Homes Framework, providing training and education to care home staff, empowering them to deliver high-quality care. The role also involves developing joint handovers in care homes to enable proactive support and prevent avoidable hospital admissions.
The DOT element of the service is currently undergoing an ICS-wide service review.
