What You Will Do: Act as the main care coordinator for assigned service users, providing screening, asset mapping, assessments, recovery planning, and care reviews using motivational interviewing techniques. Deliver tailor-made packages of care, including psycho-social interventions, key work, POD, group work, clinical interventions, and harm minimisation strategies. Conduct comprehensive service user assessments and develop individual recovery plans that support community reintegration. Foster service-based recovery communities, promoting mutual support and collaboration among clients. Perform risk assessments and manage risk effectively to ensure safety and wellbeing. Manage triage assessments and make informed referrals to treatment and community support agencies. Requirements: Updated DBS certificate registered on the update service.
