This role involves working as a rotational Band 6 Neurological Physiotherapist providing specialist assessment and treatment for patients with complex neurological conditions across inpatient and community settings. The post holder will manage an independent caseload, work as part of the multidisciplinary team, and deliver high-quality, patient‑centred rehabilitation using evidence‑based practice.


Main duties of the job

* Undertake comprehensive assessment, diagnosis and treatment of patients with complex neurological conditions using advanced clinical reasoning.
* Manage an independent caseload, prioritising workload to meet service and patient needs.
* Deliver individualised, evidence‑based physiotherapy programmes and evaluate patient progress.
* Work as an autonomous practitioner while contributing to effective multidisciplinary team working and coordinated care planning.
* Support discharge planning and continuity of care across inpatient and community services.
* Provide education and support to patients, carers and other professionals to promote rehabilitation and self‑management.
* Supervise, support and develop junior staff, students and rehabilitation assistants.
* Contribute to audit, research, service development and quality improvement activities.
* Maintain accurate clinical records in line with professional, legal and Trust standards.
* Promote the Trust's values, delivering compassionate, inclusive and patient‑centred care.


Job responsibilities

The post holder will work as a Band 6 Neurological Physiotherapist within Hertfordshire Community NHS Trust, providing specialist assessment, diagnosis and treatment for patients with complex neurological conditions across both inpatient and community settings. Using advanced clinical reasoning and a wide range of evidence‑based treatment approaches, the post holder will deliver high‑quality, patient‑centred rehabilitation.

The role involves the management of an independent and often complex caseload, requiring effective prioritisation, clinical decision‑making and adaptability to changing patient needs. The post holder will work as an autonomous practitioner, taking responsibility for all aspects of their clinical practice, including risk assessment and safe use of equipment.

The post holder will undertake comprehensive assessments of patients with diverse and complex presentations, developing and implementing individualised treatment plans aimed at maximising functional ability, promoting independence and supporting rehabilitation goals. They will evaluate patient progress using recognised outcome measures and adapt treatment programmes accordingly.

Working as part of a multidisciplinary team, the post holder will contribute to coordinated care planning, rehabilitation programmes and discharge planning processes, ensuring effective transitions of care across inpatient and community services. They will communicate effectively with patients, carers and a wide range of health and social care professionals, ensuring a holistic and collaborative approach to care delivery.

The role also includes supporting patients and carers through education and advice, promoting self‑management and long‑term condition management. The post holder will use a range of communication strategies to engage with individuals who may have complex communication or cognitive needs.

In addition to clinical responsibilities, the post holder will contribute to the supervision and development of junior staff, students and rehabilitation assistants, providing guidance, training and support to ensure high standards of practice. They may also take on delegated leadership responsibilities, including supporting day‑to‑day service delivery and contributing to team coordination when required.

The post holder will actively participate in service development, including audit, research and quality improvement initiatives, contributing to the ongoing development of clinical practice and service delivery. They will be expected to maintain accurate and comprehensive clinical records in line with professional, legal and Trust standards.

The role requires engagement in continuous professional development, maintaining up‑to‑date knowledge and skills in neurological physiotherapy and contributing to personal and team learning. The post holder will participate in training opportunities, reflective practice and professional supervision.

Flexibility is required to work across different clinical environments and service areas in response to service needs. The post holder will contribute to maintaining a safe working environment, adhering to health and safety, infection control, safeguarding and information governance policies.

Person Specification


Qualifications

* Physiotherapy degree or equivalent
* HCPC Registration
* Experience of working with neurological patients in a community or inpatient setting at Band 5 level
* Attendance at relevant post‑graduate neurological training courses e.g. Normal movement, spasticity management, splinting, cognition and perceptual assessment, fatigue management


Experience and Knowledge

* Ability and experience of carrying out a comprehensive neurological Physiotherapy assessment in order to formulate a comprehensive neurological treatment plan and goals
* Knowledge and experience of using a wide range of therapeutic Physiotherapy interventions for the treatment of patients with a stroke and other neurological conditions
* Knowledge and experience of using recognised outcome measures and goal setting skills as appropriate within neurorehabilitation
* Ability to work as an autonomous practitioner, with individuals and groups
* React in a flexible way to change demands in service
* Experience of supervising and appraising other members of staff
* Experience of supervising students


Skills and Abilities

* Physical skills, dexterity, coordination and sensory skills for assessment and manual treatment of patients.
* An ability to demonstrate clinical problem solving in complex cases
* Ability to prioritise patient workload
* Ability to manage own time and work under time constraints

* Eligible to live and work in the UK
* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
