WoolerHealth are looking for a friendly, professional, hard working, experienced practicenurse to provide extra nursing hours within our team. The post holder is responsible for the delivery and evaluation of patient care across a range of long term conditions and preventative health services including supporting health promotion, disease prevention and management of chronic illnesses in line with national and local guidelines. Effective communication, teamwork and a patient-centred approach are essential to the role.
Main duties of the job
Key responsibilities include:
Administering vaccinations and immunisations in line with PGDs and national policies, including childhood vaccinations.
Undertake long term condition reviews, ie asthma, COOPD, diabetes and managment of these jointly with the GPs.
Perform cervical cytology
Undertake annual contraception reviews
Carry out health checks, bloods and routine observations
Work alongside the admin team on QOF targets
Assist the GPs with minor surgery and long term contraception clinics
Maintain accurate clinical records and adhere to infection control standards.
Consideration will be given to applicants who have some of the required experience with additional training provided as appropriate.

Wooler Health is built around a multi-disciplinary, highly skilled and supportive team. Our clinical team comprise of 2 female partners, 3 male salaried GPs, two practice nurses, one trainee nursing associate, one HCA and one phlebotomist.
We share the Cheviot Primary Care Centre and work closely with the Northumbria team of district nurses, podiatrists and physiotherapist. The NEAS community paramedics are also based here 24/7.
We also have Well up North Network staff working with us including a first contact physio, pharmacist, mental health worker and social prescriber (dementia).
Our patient population is just under 5000 patients with 35% being over 65 years and cover a large rural area, dispensing to half of our practice population.

Job responsibilities
The Practice Nurse provides highquality nursing care to patients within a GP practice setting. The role focuses on the management of chronic disease, health promotion, preventative care, and supporting patients to maintain and improve their health and wellbeing.
Clinical Duties
Injections and immunisations (including childhood, travel, and flu vaccined
Cervical screening (where qualified)
Blood pressure monitoring
Spirometry and other diagnostic tests
Manage patients with chronic diseases such as:
Diabetes
Asthma
COPD
Coronary heart disease
Conduct health checks and lifestyle assessments
Assist with minor surgery and long term contraception clinics
Support patients in weight management and alcohol reduction
Promote healthy lifestyles and self management of long-term conditions
Maintain accurate and up to date patient records using SystmOne
Participate in audit, QOF and clinical governance activities
Manage clinical stock and ensure infection control standards are met
Contribute to policy development and service improvement
Supervise and support healthcare assistants where appropriate
Maintain NMC registration and engage in ongoing professional development
Adhere to safeguarding, confidentiality and infection prevention policies.
Qualifications
* Registered General Nurse (RGN)
* Current NMC Registration
* Evidence of ongoing Continuing Professional Development
* Practice Nurse experience including management of long term conditions
* Immunisation and vaccination certification
* Cervical cytology certification
