Overview
The Primacy and Community Mental Health Service is delivered in a mix of on site and at home working (virtual appointments). The mental health practitioner role is expected to see 26 appointments each week with 5 new patients each week. The appointments are 45 minutes long. You will provide treatment to patients which is planned to be no more than 16 sessions in length. Patients are to be seen either weekly or bi-weekly, with future booked appointments booked well in advance.
Responsibilities
* Conduct robust and comprehensive biopsychosocial assessments of need for individuals who present with a wide range of needs. This will then inform the treatment plan.
* Provide a comprehensive specialist mental health assessment and identification of a referred patient’s mental health needs, formulating and implementing nursing care plans and goals that reflect the patient’s needs.
* Work as a community mental health nurse / social worker / occupational therapist with knowledge of working with individuals who experience suicide, self-harm, acute mental illness presentations and dementia / confusion / delirium.
* Work independently in a number of environments as an autonomous practitioner.
* Regularly review caseload management with their clinical supervisor; carry out procedures in accordance with best practice and within own skill set or under supervision of others.
* Maintain accurate records about the care and treatment provided; complete patient outcome measures and encourage the completion of patient experience measures.
* Review and evaluate professional standards in conjunction with the quality team; ensure the developing requirements of the mental health professions are understood by the wider team.
* Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and service policies; adhere to confidentiality principles.
* Contribute personal and professional skills to the development of the Mental Health Team and identify and manage risks in a variety of settings.
* Offer support and advice to families and carers and act as an advocate for clients where appropriate; manage a caseload by effective use of time and resources; provide constructive supervision in clinical, managerial and educational needs related to staff development and risk management.
* Influence and implement changes to clinical practice, locally and trust-wide; participate in individual appraisal performance reviews and the construction of personal development plans.
Qualifications and Requirements
Experience and competencies include working with individuals who experience suicide, self-harm, acute mental illness presentations and dementia / confusion / delirium. Ability to work independently as an autonomous practitioner. Ensure interventions are evidence-based and align with service policies. DBS disclosure requirements are described by the employer (DBS Update Service registration is required; cost may be reimbursed through the Trust's policy; Basic DBS check requires DBS Online account).
